Vast Chiropractic & Rehab is committed to protecting the privacy of your health information. In the course of providing care, we may collect health information such as your medical and injury history, examination findings, treatment notes, imaging or referral records, insurance and billing information, and contact details.

- As required by law: for example, in response to a court order, public health reporting requirement, or valid legal request.
We will not use or disclose your health information for any other purpose without your written authorization, and you may revoke that authorization at any time.
Your Rights
Under HIPAA, you have the right to:
- Request access to and receive a copy of your health records.
- Request an amendment to your health information if you believe it is incomplete or inaccurate.
- Request restrictions on certain uses or disclosures of your information.
- Request confidential communications at an alternate location or by an alternate method.
- Receive an accounting of certain disclosures of your health information.
- Receive a paper copy of this notice at any time, even if you agreed to receive it electronically.
How to File a Complaint
If you believe your privacy rights have been violated, you may file a complaint with us directly using the contact information below, or with the U.S. Department of Health and Human Services, Office for Civil Rights. We will not retaliate against you for filing a complaint.

We may update this notice from time to time to reflect changes in our practices or the law. The current version will always be posted on this page.
